Living Well, A programme for adults with long-term health conditions

Living Well is a FREE group programme for adults living with 1 or more long-term health conditions. These include but are not limited to COPD, asthma, diabetes, stroke, heart conditions, multiple sclerosis (MS), arthritis, cancer, Crohn’s disease, chronic pain and depression.

  • It takes place over 6 core workshops of 2.5 hours once a week & is delivered by 2 trained “peer” leaders, who are also living with a long-term condition.
  • Develop the self-management skills and confidence to live well.
  • Learn how to manage things like sleep, healthy eating, physical activity, medication, communication, pain management, better breathing & falls prevention, among others.

Meet others living with long-term conditions & learn from each other’s experiences.

This online programme has an extra 30 min introductory session prior to week 1
